
What forms do I need to verify proof of address for credit dispute?

- Drivers license, utility bill, bank statement in most states.
- Ask the person on the other side of the dispute what they will accept. I'm going to assume here that this is a past address, so that you can show that some old judgment, or some other bad thing, belongs to somebody with a similar name and not you. (I've been there.) Depending on the state, your driving record may show a history of address changes. Driving records are public info and you can get a copy for a copying fee. Likewise, f your bank has records, get a letter from them. For that matter, I have my old bank statements in a filing cabinet. Pull the one for the month in question and send a copy showing the statement date and address.
